New Developments at The Lutheran Home at Kane

The Centers for Medicare and Medicaid Services announces a new program to increase quality in Nursing Facilities.  The Lutheran Home at Kane is one of one-hundred-forty-five across the nation selected to participate in a partnership with UPMC Community Provider Services.    Linda Carlson, CEO announced that this initiative in the form of a grant from CMS and is indented to ensure the best possible care for their residents, enhance the level of collaboration with hospitals and other service providers.  In an effort to help reduce avoidable hospitalizations among nursing home residents, the organization will be provided on-site services and support.  “ I am very proud that the Lutheran Home has been accepted into this program and can serve as an advocate for our clients and be in a better position as changes to the reimbursement system.”

A Welcome Note:

The Lutheran Home at Kane, a Social Ministry Organization of the Evangelical Lutheran Church of America, is licensed as a Continuing Care Retirement Community (CCRC).  The campus is located just on the edge of town in a lovely wooded area.

Over 120 residents make this their home:  whether for short term rehabilitation, long term care or just needing some day-to-day assistance.  A full range of programs are offered based on the residents individual needs, but in each case, it is important that we strive to respect and maintain individual qualities and promote a sense of belonging.

We all may face the challenges of an aging family member and look to the health care system to help us through the journey.  The process is often confusing and can be very difficult but there are people who can help. 

The Lutheran Home can be a valuable resource and a friend in time of need.  Through the organizations networks, partnerships, community connections and agency contacts, you may find some answers that may help make the journey a little easier.

Elder Justice Act - Reporting of Abuses and Crimes
It is the policy of The Lutheran Home to notify board members, employees, consultants, contractors, vendors, volunteers and other individuals of their duty to report reasonable suspicions of crimes to the elderly. While the Lutheran Home has policies and procedures in place to prevent abuse, complete background checks. investigate abuse, in-service employees and report to the Area Agency on Aging and Pennsylvania Department of Health, some crimes under the EJA also require reporting to law enforcement. Policies and procedures have been updated to comply with the new EJA regulations. • Make sure that medical/mental health information that identifies you is kept confidential;
• Give you this notice of our legal duties and privacy practices regarding your medical information;
• Follow the terms of the notice currently in effect.
